Simple Email Form React JS + Bootstrap

Hi,

I’m using netlify forms for the first time inside our react app. I’ve followed the available instructions but it is not working. Netlify see there is a form but the email is not being received and we are taken to a 404 page. Here is the setup, sorry for formatting. Thanks in advance!
