Netlify form not submitting, or showing page not found error

Hi everyone,
I have research this issue and done everything they say bu I still dont get either the captcha or a form submission. Everytime I clicked on the submit button I get this error-

Page Not Found

Looks like you’ve followed a broken link or entered a URL that doesn’t exist on this site.
