Is there some way to have an individual @ mentioned on Slack when a form submission comes in? I tried just adding @Carl Johnson to a form that goes to a general Slack channel, but I did not get a notification from that.

I don’t believe that’s possible, however I think you can customise Notification Keywords in Slack. So you could get notified for some specific keywords which should help you achieve what you need.
Slack keywords don’t trigger alerts if the keywords are said by a bot, unfortunately. To do this requires building out a whole Slack bot that can DM specific people, which is more work than is worth it for my feature needs.
